How to read a titration schedule

The 2.5 mg starting dose exists so the body can adapt to slower gastric emptying. It is a tolerability dose, not a treatment dose, and judging the medication on how the first month feels, in either direction, is judging it on a dose never designed to do the job.

Four weeks is the minimum before a step up, not an obligation to take one. Prescribers routinely hold someone at a step for longer when side effects are heavy, and that is a clinical decision rather than a delay.

Maintenance is commonly 5, 10 or 15 mg. Reaching the highest strength is not a marker of success, escalating past a dose that is already working adds side-effect risk without a guaranteed additional benefit.

A single-dose pen delivers the stated dose with nothing to work out. Draw the same medication from a vial and you are converting milligrams into syringe units, which is where dosing errors actually happen.

How do I convert a Zepbound dose into syringe units?
A single-dose pen needs no conversion. If you are drawing from a vial, divide the milligrams in the vial by the millilitres of liquid to get concentration, divide your dose by that, and multiply by 100 for units on a U-100 syringe.
